                                          Cambodian Senior Case Management Services

                                          Cambodian Senior Wellness Program/Social Services Grant

                                          This project provides Cambodian seniors with culturally and linguistically appropriate psychosocial activities and services, including Case management, exercise programs, informational presentations and discussions, health promotion and educational seminars, classes in survival English, Khmer cultural activities, nutritious meals and referrals to a continuum of health and human services including food stamps enrollment, Medi-cal enrollment, counseling and mental health programs and education.

                                          This program is funded by the City of Long Beach.

                                          Health Access for Pacific Asian Seniors (HAPAS)

                                          HAPAS focuses on capacity building and community health education regarding adult immunizations, cardiovascular disease and diabetes. Its goal has shifted towards policy change in physical activities and healthy diet. A Walking Group has been created based on the results of our community focus group, in hope that this walking group activity will lead to community acknowledgement and recognition of the beneftis of exercise and health diet, which could eventually lead to more advocacy for policy change.

                                          Supportive Services Program (SSP)

                                          SSP offers case management services to Cambodians who are 60+ years old. Services range from assistance with doctor's consultations, bicultural services (translation), medication support, housing, family counseling, socialization, linkages to various community resources and advocacy.

                                          This program is funded by Special Service for Groups.

                                          Family Caregiver

                                          The Family Caregiver Program is comprised of four components: community education, training, support groups and case management. The education and training covers understanding the roles of caregivers and available caregiver resources. Case management is for those clients who need additional individual support. Support groups offer additional support and networking to current caregivers.
